defeating the Pride demon at the end of the prologue.
You should be able to open the chest, marked "Special Deliveries" or some such, immediately after the cutscene with the nervous elf.
ETA: All of the Flames gear is named "---- of the Dragon". The class of armor (heavy, medium, light) depends on your class (warrior, rogue, mage), and you only get the actual weapon(s) for your particular (sub-)class. For example, 1-handed warriors get their sword and board, rogues get (one! <_< ) dagger, etc. For the rest, rumor has it that you should get their schematics at some point (haven't gotten there yet myself, apparently), so you can craft them.

Other users have reported that the mounts become available as War Table missions after a certain point. (I would think you'd have to get the stable first, otherwise you'd have nowhere to put them!  But that's just a guess.)
I'm afraid I can't help you with the MP chests, as I'm SP only. Sorry! :unsure:
